Annual Care Visits:
Goal is to have 70% of your patients receive a wellness or preventive visit in 2015. Are you on track?

3 Months remain to schedule your patients for their annual care visits.

• For Commercial members above the age of 50, all attributed members who were attributed for the full 12 months of contract period will be used in the measurement of this quality measure. A member will have been considered to have met the requirement if the member has seen a PCP for a well or preventive visit which is determined based on the following codes: (CPT Codes 99381-99387, 99391-99397) in a one year period.
