The transformation of health care in this country will be driven by a transformation which delivers high quality, efficient primary care. To accomplish this, the IPA believes that adoption of the PCPM is necessary. We have dedicated resources in this process and engaged Physician Leaders (PLs) to create this transformation.

The PCPM is the CSMS-IPA model for addressing the changing healthcare environment with a "Medical Home" management approach to care. The CSMS-IPA Patient Centered Practice Model (PCPM) network is currently comprised of 1021 Connecticut physicians (both primary care and specialists) throughout the state.
Components of CSMS-IPA’s Patient Centered Practice Model (PCPM)
- Alternative Payment Models – These are necessary to support the investment in resources, financial and human, needed to accomplish the shift to a PCPM. The IPA is experienced in both risk and non risk contracting and can manage the process.
- Education-CSMS-IPA has partnered with the American College of Physicians (ACP) to assist primary care physicians in building capacity to become a PCPM. A free license to ACP’s Medical Home Builder is available to all primary care physicians who opt in.
- HIT – The IPA provides physicians who wish to embrace this concept with appropriate HIT with little to no financial risk.
- Case Management - Trained Managers assist in the care of complex, chronic disease patients with the intent of dramatically reducing ER visits and hospitalizations.
- Improved Communication – Enhanced patient communication through the use of secure web-based portals.
- Team Building – Assistance in the development of care teams within a practice to improve access and quality.
- Patient Educational Modules - To promote self care and wellness.
- Performance Feedback Data – To keep physicians informed as to their progress in the PCPM regarding cost of care, referral tracking, and hospitalization utilization
- Emergency Room Utilization Tracking - Communication with emergency facilities to promote office services rather than ER visits.
- HCC Coding Review and Education
- Imaging Management – The use of a web based tool to educate and promote the appropriate choices for ordering imaging studies.
