Transition Care Management Codes
Two new codes have been added for 2013 for Medicare and Medicare Advantage patients being discharged from inpatient facilities at hospitals or Skilled Nursing Facilities.

Incentive Payment for PCMH
It has come to our attention that primary care practices that have achieved NCQA Patient Centered Medical Home (PCMH) Recognition are receiving additional per member per month (PMPM) fee from some of the payers. This payment is in consideration of the care coordination provided by PCMHs and applies to selected commercial lines of business. We encourage you to contact your payer representative to see if you are eligible for a financial incentive program for PCMH recognition. CSMS-IPA has been active in PCMH training and adoption.
