JOB SUMMARY:

The care coordinator will work in the setting of Patient Centered Medical Home practices in support of identifying patients at risk and in need of care management intervention. This person will be responsible for coordinating care for those patients and assisting in the execution of care plans promoted by the IPA and/or a payer. It is anticipated that the care coordinator will have access to patient records and the patients. The care coordinator will promote wellness through patient education and monitored compliance with a physician treatment plan.

QUALFICATIONS:

• Registered nurse, Licensed practical nurse, or Social worker with current unrestricted license in the State of CT
• Minimum of 2 years experience in care/case management
• Experience working within a physician practice with direct patient contact preferred

DUTIES and RESPONSIBITIES:

• Validate Health Risk Assessments (HRAs) via chart review
• Obtain HRAs if not submitted
• Assist in the identification of high risk patients appropriate for care management and coordination of services
• Promote high-risk patient recruitment into existing IPA or payer care management programs
• Expedite care transition communication
• Assist with the ER avoidance program
• Contact with patients at risk to maximize wellness and avoid inappropriate hospitalizations
• Provide regular updates to evaluate outcomes
• Maintain positive, professional office relationships
• Maintain patient/physician confidentiality and professional standards
